Annual Employee Performance Review

1. The employee and supervisor both have an opportunity to review this form prior to it being placed on file at the RHD office.
2. The reviewer should check the single statement that best describes the employees performance for each evaluation factor. Comments and specific examples are required if the rating of “does not meet expectations” is checked.
3. The employee may add remarks in the “Employee’s Comments” section.
4. The supervisor and employee should discuss objectives for the upcoming year.
5. The employee will receive a final copy of the completed form.
