This document provides an example of the annual performance review.

Performance Review Metrics

  1. Role Model / Exceeds Expectations
    • Definition: Consistently delivers work of the highest quality, rarely making errors. Demonstrates initiative and provides value beyond their core responsibilities.
    • Description: This employee is a role model for others, proactively solving problems and suggesting improvements. Their work often surpasses expectations.
  2. Solid Contributor / Meets Expectations
    • Definition: Produces high-quality work with minor, infrequent errors. Dependable and reliable in fulfilling job responsibilities.
    • Description: This employee consistently meets the requirements of their role, contributing solidly to team objectives and maintaining steady productivity.
  3. Needs Improvement
    • Definition: Meets the basic requirements but tends to stick to the minimum standard of output and effort, with occasional errors.
    • Description: This employee completes tasks as assigned but requires motivation or additional supervision to improve quality, consistency, and engagement. There is potential for growth with guidance and support.
  4. Does Not Meet Expectations
    • Definition: Work quality and/or quantity falls below acceptable standards, with frequent or critical errors impacting the team.
    • Description: This employee does not meet the expected standards and requires improvement in several areas. Regular feedback and performance management are needed to achieve a satisfactory level.

  • Produces work of the highest quality, rarely makes errors.
  • Produces high quality work with a few errors.
  • Produces acceptable work with a tendency towards errors.
  • Produces unacceptable work with excessive errors.

SECTION 2: QUANTITY OF WORK

  • Meets deadlines ahead of schedule, then seeks additional tasks or assists others, recognized as a peak performer.
  • Completes most deadlines ahead of schedule, above-average output levels.
  • Meets deadlines within specified time limits.
  • Does not meet deadlines within time limits.

Comments and examples:


SECTION 3: INITIATIVE

  • Constantly exhibits independent action and resourcefulness; exercises outstanding judgment on knowing when to seek guidance from others, highly motivated team member.
  • Frequently exhibits independent action and resourcefulness, exercises good judgment on knowing when to seek guidance; above average worker.
  • Generally demonstrates independent action with minimal supervision; dependable worker.
  • Rarely exhibits independent action as required by the job, requires constant supervision.

SECTION 4: INNOVATION / ADDING VALUE

  • Exceptional contributions to the team, consistently seeks ways to improve work methods, offers constructive and detailed positive suggestions.
  • Develops methods for streamlining operations, frequently provides constructive suggestions.
  • Develops better methods of completing work, occasionally provides constructive suggestions at meetings.
  • Requires supervision to work efficiently in the unit, does not provide constructive suggestions.

SECTION 5: TEAMWORK AND CUSTOMER SERVICE

  • Constantly anticipates and meets the needs of customers. Always communicates and works well with co-workers and customers.
  • Frequently anticipates the needs of customers, generally works well with co-workers. 
  • Meets customer needs, communicates and works acceptably with clients and co-workers. 
  • Frequently fails to work positively with co-workers and/or clients. Needs improvement with customers and/or co-workers.

SECTION 6: JOB KNOWLEDGE

  • Demonstrates exceptional knowledge and skills in the most complex aspects of this job. Understands and follows company policies.
  • Demonstrates better-than-average knowledge and skills in handling complex job duties.
  • Demonstrates adequate knowledge and skills to perform job duties
  • Lacks knowledge and skills necessary to perform certain job duties.

SECTION 7: LEADERSHIP

  • Influences, supports, and motivates customers, staff and co-workers. Leads by example; makes timely and productive decisions; uses resources wisely; delegates effectively; always supports company values and vision. Participates and has leadership roles in the industry and community.
  • Encourages the development of employees and/or co-workers; promotes department initiatives; makes effective decisions; uses most resources wisely; often delegates effectively; frequently contributes to activities supporting company vision and values. 
  • Supports the progress of staff and/or co-workers; makes good decisions; generally uses resources wisely; contributes to meeting the company vision and values;
  • Occasionally neglects staff and/or co-workers needs; makes poor decisions for the unit; uses resources poorly, deflects responsibilities and accountability to co-workers.

  • Excels in open communication skills; leads others to support work-related responsibilities; promotes a supportive work environment; always treats others with dignity and respect.
  • Fosters open communications; encourages co-workers to contribute to the success of the unit; provides a supportive work environment; demonstrates how others should be treated with dignity and respect.
  • Demonstrates acceptable communication skills; appropriately responds to others; treats people with dignity and respect.
  • Demonstrates poor communication skills; does not respond appropriately to others; creates extra burdens for co-workers, discourages others; fails to treat people with dignity and respect.
